This position functions as a case manager for client families in the home setting, providing appropriate support services which includes access to legal, physical and mental health services.   Working with intergenerational family units to create case plans and family goals, the case manager assists caregivers and families keep Department of Social Services, school and probation appointments and provides assistance with the foster care interviews and juvenile court sessions.

Minimum Requirements

• Familiarity with the needs of relative caregivers
• Good oral and written communication skills
• BA is preferred 
• Proven problem-solving skills
